Registration Process and Requirements

Once you have made the decision to register your child at Providence, an online registration package will be emailed to you. In addition to the registration form, you will need to submit the following documentation:

• Your child’s birth certificate. If your child was not born in Canada, please provide a copy of Permanent Resident/Landed Immigrant status/Child of a Canadian Citizen.
• Copies of your child’s therapy reports, assessments and/or a doctor’s letter stating your child’s medical diagnosis.

Once you have completed all registration requirements, Providence will apply for Program Unit Funding (PUF) for your child.
